Please mail your registration form and check, made payable to Play in the Mud Studio to P.O. Box 5231, Madison, WI 53705-0231. You will receive your confirmation via e-mail.

Refund/Cancellation Policy: If you must cancel a workshop, every effort will be made to fill your space. If the space is filled, we will return your class fee, less 10% for administrative costs. No refunds will be given for cancellations received less than 30 days before a workshop.

I understand that the registered individual participates in art classes at his/her own risk. I will not hold Play in the Mud Studio, its owners or employees liable in case of accident or injury to registrant's person or possessions.
